Answer: yes, they will likley cut in the same location as the existing scars. The areola can also be made smaller. It will help greatly if you can get a copy of the operative report for the new surgeon so they know what previous technique was used to limit risks. Down time is about 2 weeks light duty and then full activity in one month, fees vary from surgeon to surgeon though so you need to decide on the surgeon and then get the quote. Answer: I think it is likley safe to do so. I think Dysport starts working faster and likely lasts longer. No agent like Dysport or Botox will correct lines seen at rest though, migth need a filler in addition to them. JG 7.3.09

Answer: no, tattoed skin will not keep all of the color and will likely not have even color either. I personally would keep as much of your own tissue as possible and not remove any more tissue or skin. JG 7.2.09
Answer: I have personally not had a permanent loss, and usually by 6 weeks things are epretty good, keeep massaging to help the swellling go down and that might help get the nerve functioning better faster, all the best. JG 7.2.09

Answer: grafts will never look as good as the natural areola that survived. The color may not match but that is one of the limitations of the procedure that cannot be surmounted. Graft reconstruction of the areola, like any skin graft, typically is not a completely uniform color, there wil be light and dark areas depending on how well the graft heals. The ability of grafted skin to accept tattoo pigment is variable and unpredictable but in most cases the color takes, tattooing the potential skin graft will not help in any way. I think you need to come to the understanding that your areolas and nipples will never look totally natural in shape or color, no matter what you do to reconstruct them, sorry. JG 7.2.09
Answer: as to the electrolysis, what I have seen is that it leaves small darker spots. As to the Botox, I am afraid that any effect from that is shortlived and in time, as the Botox wears off, what was going to happen without the Botox still happens. As to the results from a scar excision and revision, it is all a matter of skin tension, if the skin is loose, it might leave a good degree of improvement. If it is tight upon closing the wound, then the result might look exactly the same as what you have at present. JG 7.1.09
Answer: no, becasue all of the tissues are above the access plane and none of that would be able to be seen. No way to correct a festoon endoscopically. JG 7.1.09
Answer: I do not think it will help the lumps if they have been there that long, they will likely need to be cut out. JG 7.1.09

Answer: Not really a a double bubble, it is a crease where the muscle ends and the breast tissue begins, it happens not infrequently with below the muscle augmentation and in most cases gets better on its own, if not, the muscle migt have to be cut a little to let it relax and not have the indentation when it flexes. It should not hurt your implant though. JG 6.30.09

Answer: I do not inject Juvederm there but I have had Restylane stay in place for longer than a year in a small percentage of patients. Now when a lump forms, it is very common for it to last long periods of time because the surface is smaller and usually encased in scar tissue making the dissolution of it by your own body`s enzymes slower and more difficult for it to work. Answer: likely just the skin where the Juvederm was injected that has to slowly shrink back to its normal appearance. JG 6.29.09

Answer: we will be able to use Dysport in July. As to chepaer, that is up to the doctor and not the manufactuer. I will persoanlly stop using Botox in most cases. JG 6.29.09
Answer: well the nerve stimualtion while the nerve is coming back will help prevent the muscle from atrophing and I think it is worthwhile, we use home units the patient can use a few times every day to make it more convenient. The casues of the scars widening are myriad and silicone sheeting migth help some at this point. The redness should improve over 6-8 weeks though. I will say that not seeing the surgeon for one month after the procedure is unusual, usually I see the patient the next day, at one week and then 2-3 other times during the first month and the pateitns are having ultrasound massage in the office twice a week during that time so if there is a concern I can see them more frequently. I think it will all get better but it will not be overnight, hang in there. JG 6.29.09
Answer: silicone or other material implants can erode into the bone, the number of Medpor implants used is much smaller so we really do not know if they have the same effect. I think if the size change she is going to go through is dramatic, there can be some space but in most cases it is not palpable or visible as there are tissues of adequate thickness over the area. I do agree that taking out large implants and not replacing anthing can leave depressions but in most cases if something new is placed that is not a problem, even if the new ones are smaller. The Medpor impalnts are more difficult to place and in most cases there is a fixation product used to avoid it rotating. I wish her the best. JG 6.29.09

Answer: if the skin is loose, yes, as to the Botox, it is a waste of time as the Botox will wear off and what was going to widen will widen. As to the electrolysis, it might leave dark spots where the hair was. JG 7.1.09

Answer: as to fillers, no, as it will not stop any naturally occuring process but using products like Botox or the newer, and I feel better, Dysport, that weaken the muscles so the skin cannot fold as much can. The reason thay can is because some of the wrinkles that form occur from the repeat folding of the skin and resultant skin damage, so if the skin does not fold as much, the development of the wrinkles will be less. Of course, if you just voluntarilly move the muscles less, that will also work, especially when trying to avoid squinting. JG 6.25.09
Answer: your previous question was in regards to the jowling I believe and in that case, a traditional facelift will do two things, lift sagging tissues and then redistribute the skin in a natural appearance, the limitation of a limited access procedure like a MACS is that while it works well in younger patietns with minimal to no skin excess and just sagging tissue, in an older individual, it can at times leave the skin redraped in an unnatural fashion leaving the stigmata of a facelift procedure. Answer: I think for cosmetic reasosn I would not recommend it, I would do it for patients after a burn or scar tissue. I am unsure whether they could remove the extra eyelash and include the hair bulb for it to work. JG 6.23.09
Answer: I think that after two years the hyaluronidase will not help, that is for the soon after time. You might have what is called a granuloma there and at the point might need to be cut out, sorry. JG 6.23.09

Answer: Fraxel does not really cause break outs but it does injure the skin and then the skin needs to heal itself by generating new skin and the skin has to produce oil to do that, and that can cause new pimples or blocked pores to form. I think Levulan for acne is a reasonable option, and in experienced hands is safe, yes. Answer: the study you mention does document lipid peroxide elevation, but that is how all ablation works, you cannot remove something without oxidatie damage so it should not be a suprise, it should be expected. Also, no thymine dimer elevation is good. Any method, including surgery, causes oxidative damage for a period of time, but that damage related here shows no correlation to potential fat loss in IPL done in isolation. I would feel comfortable with IPL treatments. JG 6.12.09
Answer: complications are rare, less than 1-2%, the issues might be too much skin being removed and the eyelids not closing well, or an alteration in the shape of the eye.
