In the early days of cosmetic rejuvenation procedures, options were limited. Other than ineffectual creams and exercises, the only real treatment to counteract the effects of gravity, sun damage and aging was a facelift.

Fortunately, we now live in a time when facial rejuvenation can begin at almost any age and any stage, thanks to a myriad of new techniques and products that allow you and your facial plastic surgeon to take control of the aging process. Chief among any cosmetic surgeon's anti-aging armamentarium is an array of facial injections that can smooth out wrinkles, replace lost volume, melt away double chins in a series of nonsurgical procedures that will keep the clock turned back longer than ever.

BOTOX® Cosmetic

The original and most well known of the cosmetic injectables, Botox is a neuromodulator made from minute amounts of purified botulinum toxin. Botox is injected directly into the dynamic muscles that cause troubling frown lines (also known as “11”s or glabellar lines) and crow's feet.

Botox works by blocking the signals the nerves usually send to the muscles that force them to contract. The contraction is what folds the skin and can cause a wrinkle if the skin has begun to lose suppleness, volume and elasticity due to aging, genetics, and/or sun damage.

Once the Botox takes effect, the dynamic muscle relaxes and no longer contracts or does not contract as fully. Without being pressed and folded by the underlying muscle, the skin smooths out and the wrinkle disappears or fades considerably.

Botox treatments only take 5 to 10 minutes in the doctor's office. They are relatively pain free, though you may experience some temporary bruising, soreness or redness at the injection sites for a few day.

You may start to see some of the effects of Botox within 48 hours after receiving your treatment, but it can take up to two weeks for the neuromodulator to fully relax the dynamic muscles so the skin can smooth out. Even after Botox has taken effect, if your wrinkles are very deep they may still be visible. In such cases, Dr. Garcia will then recommend adding in one of the dermal fillers to even out your skin and remove the wrinkle.

Botox is gradually metabolized by your body and will need to be reapplied after three to four months, when its effects begin to wear off. As you continue to use Botox, the treated muscle may be relaxed enough that it needs less and less Botox to keep your skin wrinkle-free, but this will vary by individual.

Botox is only FDA approved for treating glabellar (frown) lines and crow's feet. While some doctors inject other dynamic or even static muscles with Botox, Dr. Garcia believes the risk of muscle paralysis or weakness that could cause distortion or temporary disability is too high. If you have wrinkles that cannot be treated with Botox, Dr. Garcia will have other recommendations for you.


Dysport is another neuromodulator that uses a purified form of botulinum toxin to block nerve signals to the muscles. It is FDA approved to treat only the deep vertical frown lines (i.e., glabellar lines or “11”s) found between the brows in adults who are younger than 65 years.

Dysport is particularly useful in cases where frowning has created an angry or concerned expression that does not resolve even when the patient is consciously relaxing his or her face. Like Botox, it is injected directly into the muscles during an office visit. Treatment takes just a few minutes. Results may begin to appear within 48 hours, but full results will not be visible for about two weeks. Dysport will also need to be readministered every three to four months. The amount needed to create the same effect may diminish with repeated use, depending on your metabolism.

Dermal Fillers

The dermal fillers are made of biocompatible materials that can add volume to areas of your face that have lost fullness due to aging. The dermal fillers are an excellent, safe, nonsurgical way of temporarily resolving facial wrinkles and folds, such as nasolabial folds and marionette lines. Depending on which dermal filler you and Dr. Garcia choose, your results can last anywhere from 4 months to 18 months or more. The dermal fillers have the added bonus of stimulating the production of new collagen, which helps your results last longer.

Some of the most popular dermal fillers are made from hyaluronic acid – a natural substance that is found in our skin as a moisturizing and plumping ingredient.

Examples of hyaluronic-acid dermal fillers include:

Juvederm – Smooths out wrinkles such as nasolabial folds and marionette lines. Results last from 4 to 6 months.

Juvederm Voluma – Smooths out wrinkles and can be used to add volume to the midface and cheekbones. Results can last up to 18 months.

Restylane – Smooths out wrinkles and can be used to augment lips. Results last from 4 to 6 months.

Restylane Perlane – Evens out deeper nasolabial folds and marionette lines

Other dermal fillers include:

Artefil – Composed of collagen and polymethyl mehtacrylate microspheres that work as a scaffolding system to support the new collagen your body produces. Artefil corrects smiles lines and fills out scars. Results can last for years, not months.

Radiesse – Radiesse is composed of calcium hydroxylapatite microspheres suspended in an aqueous gel that stimulates the production of your own collagen. Radiesse is [articularly effective in restoring and rejuvenating the hands. Results can last a year or more.

Sculptra Aesthetic

Scultra Aesthetic is a new type of facial injectable, made from poly-L-lactic acid, which gradually replaces collagen lost due to aging and stimulates your body to produce new collagen. It can be used to fill in deep troughs and wrinkles, such as nasolabial folds and marionette lines, plus restore contours and fullness to the midface and cheekbones,creating a nonsurgical mini “facelift” in some cases.

Sculptra requires three treatments over a 3- to 4-month period. Results can last up to 2 years.


Kybella is the only FDA-approved injectable that can dissolve submental fat, more commonly known as a double chin. Kybella is composed of deoxycholic acid, which is a component of bile, so it is biocompatible and easily tolerated. It is a safe, effective, nonsurgical alternative to facial liposuction of the chin.

Kybella can be administered in the office in 5 to 10 minutes. You will need at least two to three sessions to get optimal results. Because your body must naturally and gradually eliminate the fat cells that Kybella destroys, it will take about 3 to 5 months before your final results are visible. With proper diet and exercise, your results should last years to a lifetime.

While Kybella works best on patients who have good skin quality, if you are at risk for sagging, Dr. Garcia can pretreat your skin with laser toning. This will help tighten your skin so it can adjust to the loss in volume once Kybella has removed your submental fat.
