Answer: it is difficult for me to say whether all of these things could be done without seeing you, the nipples and some skin could be transplanted to another part of your body and then moved back in time but the retention of the grafts is not always good. As to why the tattoo color did nto take, that is not uncommon as grafted skin has dense scar tissue and that can make it hard for the color to hold fast. Answer: if the area is small and the surrounding skin is loose enough, it is possible, if it is too tight, then the incision will widen with time and reveal a spread out scar. There are no real benefits to the skin graft, it is used when there is not enough normal surrounding skin to correct the problem. The top part could be removed, but how to close it would need an inperson consultaton to determine if there is enough skin to close it. Also, if the loss is at the top part of the areola, trying to close it with surrounding skin wil pull the nipple itself into a higher position, something to consider. JG 10.31.08

Having a baby does not mean you accumulate fat inside of the abdomen, a healthy pregnancy only needs about 25 pound weight gain so if you keep to that there should not be much extra fat anywhere. Liposuction can only remove fat from below the skin, nothing below the muscles, so whether fat can be removed with liposuction depends entirely on where it is located. JG 7.23.08
Answer: I think revison rates of about 5% are the national norm, if it is something that I feel did not heal well there are times when I will do the revision at no charge, there are times when I feel all healed well but the patient wants something more and in those cases I might waive the surgical fee and only charge the operating room costs and the anesthesia costs. If the patient has aged or had expections that were not reasonable, then I would charge the patient again, all depends, no set answer. I have had patients that after one or two surgeries look tremendously better, to the point that their friends think they look great and they are still not happy. Happiness is inside of the head and what we do as plastic surgeons is psychiatry with a scalpel, some pateints, no matter how they look will not be happy, and some patients no matter what physical deformity they have are ecstatic. Answer: I think that would be a mistake, it might lead to yet a larger keloid on the chest and a keloid at the site where the graft was taken from, could consider re-excision of the scar followed by XRay therapy. JG 7.23.08

Answer: the fluid collects there because there is a watershed area where the muscles of the areas do not completely abutt to on another causing a small seperation that allows the fluid to accumulate there. The injections I am afraid are not permanent. I do not think kenalog injections ar a good long term option due to potential tissue loss. JG 7.13.08
