- Dr. Garcia, I had sinus surgery last April of 2011. I am still feeling pressure at times in the ethmoid region. Nothing constant. I recently came across some information pertaining to a herb called ganoderma mushroom (i.e. reishi) and its many healing properties to include allergies and sinus congestion. Have you heard and/or had any experience with this herb and its ability to help with the sinuses? How much longer can I anticipate the on and off pain?
Answer: Impossible to tell how much longer the pain will continue, you might need the ethmoids opened again. i think taking the Reishi is worth it, some people do better using it. All the best. jG 2.21.12
- Good morning. Today the fluids he removed were similar to yesterday and I can tell she was disappointed. I am wondering if you know of anyone in our area for a second opinion. My right side is progressing nicely but any sort of food seems to make the fluid accumulate on the left. She suggested a tube through my nose for nutients but I think I would just swallow more and be irritated, let alone scared. I am very healthy and at a good weight losing a little would not be a problem I know as I have always been a stable weight. I am in the hyper chamber twice daily and sipping water and milk and an ensure for diabetics. I know she would do anything necessary but she keeps saying this is a fluke. Have you ever experienced this? Do you think its time for 2nd opinion? There was a bit of this happening on my right in the beginning but it resolved itself very quickly. I am now a full week post op . I was very impressed at your immediate suggestion of what it might be. Thank you.
Answer: the fact that the fluid accumulates when you eat points to a salivary fistula. Drugs such as atropine or glycopyrrolate will decrease saliva production and do help quite a bit.I think placing a feeding tube is overkill. A drain being placed will get the skin to stick down and scar over the puncture wound in the partotid gland, and along with the glycolpyrrolate, it will close. The combination works better than one of them alone.It is a rare occurrence, but can happen. I have helped patients that came to me with that problem with the protocol I have outlined above. I think if your surgeon does not wish to do those things, you might want to get a second opinion for your parotid fistula. The other resort is to use radiation therapy to close it, but it will likely leave some permanent damage to the saliva gland that can affect how your food is digested and it`s taste. All the best. - If one has laser hair removal, what are the best lasers for that? Is the gentle YAG one of the better lasers for hair removal?
Answer: I feel an Alexandrite laser is very good, the YAG is good, but not as good as the Alexandrite, in my opinion. JG 2.21.12
- So is it feasible to do a PCA peel series (excluding the rosacea areas from the peel) at the same time as a series of IPL, alternating the treatments, each 2 weeks apart?
Answer: correct, 2-3 weeks apart, as tolerated. JG 2.21.12
- So in my case where I struggle with both rosacea and melasma, is IPL my best option as it can address both? Also, can I have two passes with the IPL? - one pass at settings to address melasma and one pass at settings to address rosacea? Thanks for your help in this.
Answer: it is part of the regimen, you will need to use more than one thing though, ie retinoids and hydroquinones only on melasma areas, topical antibiotics for the Rosacea, along with sunblock. Double pass at different settings is possible, yes. JG 2.21.12
- Hi Dr. Garcia, Thank you for your advise. So if I understand correctly, IPL may worsen my melasma initially but will help to lessen it in the long term?
Answer: correct, although Melasma is deep, so it is better treated with a YAG laser, which is a true laser where an IPL is an intense light, not a laser.JG 2.21.12
- Well, here`s a new dilemma for you, Dr. Garcia! Last week, I was finishing a SkinMedica Rejuvenize peel, stretched out on the table and cooling off, when a vial of pigment for permanent make-up, which was on a shelf above the table, fell down (when someone in an adjoining office closed a door too firmly) and scattered its contents all over my face. The aesthetician couldn`t wipe it off with saline, so she scrubbed my face with alcohol to prevent the pigment from staining my skin. She claimed that because the chemicals in the peel had already absorbed into my skin, the alcohol couldn`t wipe them off or dilute them; in fact, she told me I would get a better peel because the alcohol would act as an additional drying agent. Does this sound right to you? One week later, and I haven`t peeled at all. I know that some people don`t peel that much, particularly if their skin is already exfoliated (as mine always is from Retin-A), but I`m wondering if these folks owe me another peel. What do you think? Thanks so much, Dr. Garcia!
Answer: I know of no medical reason why a drying agent like alcohol would make any peel better if applied after the peel is done, before to degrease the skin, maybe, after, no. Not sure they will give you another peel though because, as you said, although its called a peel, peeling does not always go with it. Sorry. What was a vial of permanent makeup doing open on a shelf anyway?!JG 2.21.12
- Hi dr Garcia. I went to the ps today and was pleased that finally the fluid was about half of what it was yesterday. I am using a motion sickness patch as well as benadryl, and very bland Liquid diet. The swelling us down. She was planning put in a drain today but did not because of reduced fluid. I feel much better and in no pain. My question goes to the healing timeframe for this injury to the salvary gland. Should it self heal? Everything else is really good and expertly executed as far as I can tell. I had a hyperbaric chamber session prior to surgery at her recommendation. It was very hard to clear my ears and even when I woke from surgery I told her my ears felt full of water. Could this have irritated the gland? My concern for healing is less stressful today but I am still in slight panic mode and don`t want to do anything to compromise my surgery. Thanks for this wonderful blog
Answer: It should close on its own the hyperbaric will not hurt and may help it actually. Stay on bland foods and avoid tart things like lemons so the gland works less. All the best. JG 2.20.12

Answer: Still at work,patients take advantage of the holidays to come into the office. I think the Sculptra is likely safer, but the correction is not super dramatic though and does not last as long as it doe sin the face. JG 2.20.12
- Hi Dr. G., I have had leg vein treatments but of course, they come back. While they are not major, I was wondering if I would have better results with laser vs. injections? What are the advantages/disadvantages of both treatments? Is laser more expensive? Thank you!
Answer: I believe the lasers are no better, and in some cases worse than the injections, due to the heat and potential scarring. Laser costs about the same, but neither one lasts any longer than the other. JG 2.20.12
- Dr. G, you`re the best. I needed to follow up on my question (dated 2-16 *below* re: eyelid surgery). I`m a bit confused. What I`d first like to ask is how is it possible that - even though I have a scar which means my PS did surgery - can my eyes look minimally improved? (My mother and sister actually say they look exactly the same). How can this be? Here in NJ, I spent a LOT of money on this upper/lower eyelift and its upsetting. Honestly, I`m thinking of asking for my money back (though I don`t really know how. Too late to do a dispute on my credit card). And truthfully, I don`t want to do this. I`ve had other surgeries with this PS and they were good. The ONLY option he`s given me is doing an incision off to the sides of my eyes. He said he`d put the scar in the crow`s feet - but (believe it or not, at 46, I don`t even have any crow`s feet so I`m not sure why he even said that - perhaps he was assuming crow`s feet are a coming eventuality at my age?) When I smile I have a slight crinkle but that`s all. Nothing if I don`t smile. You mentioned (below) something about *not* excising any skin. How do you not excise any skin? I`m assuming I`m getting a local (in his office, ugh) and he will do a small incision and then tighten up the corners. He does not want to do any more surgery directly under the eye. Would you say this is right on his part? Thank you!!!!
Answer: as to how it can look the same, it might have been minimal fat removal or an inadequate release of the fat pockets and the sutures that were placed to hold the fat did not hold and broke loose. As you have had surgery by the surgeon before, I think being frank with him is the best approach. You should explain to him what you saw before the surgery and now, and ask him how he proposes to correct what is there now from only a crows foot incision. If he can do all of that from the crows foot incision now, why could he not do it that way before. Wish I could suggest more. Thanks for the kind words and I wish you the best. JG 2.20.12
- I had a FL/NL 4 years ago with good results except my PS did not fat graft. I have a tiny face and really needed volume. I recently had restylane (Jan 31st) and it looks awful-my jowls are back and the texture of my skin looks dry and saggy. Did the restylane ruin my facelift? Dr did a little vitrase to reduce some overfill but it still looks watery and saggy-he also gave me diuretics for 5 days-I am beside myself. All I wanted was a little volume. I look worse than I did BEFORE my fl. WIll the skin go back to normal? Fluid in the jowls dissipate? I also have a small spot of resty that migrated under my lip and I look like i have a lump. This was a reputable PS!!! Any suggestions?
Answer: I do not think the Restylane ruined your facelift, it sounds like either you developed some swelling around it or it was injected into the wrong area. Diuretics will not help I am afraid. The filler needs to dissolve and the fluid absorbed. The skin will go back to normal in time, yes. The lump on the lip can also be injected with the hyaluronidase too. Hang in there.JG 2.20.12
- Hi Dr Garcia, Me again, the melasma and rosacea gal. In regards to my questions, I should let you know that I am a Fitzpatrick III, and I use zinc/titanium sublock spf 30 daily rain or shine. Thanks again for your help!

- Hi Dr Garcia, I am lucky enough to have melasma and rosacea (subtype I) at the same time :-) I am currently treating the melasma with tretinoin and 4% HQ and will start a series of PCA peels this week. The results have been wonderful so far. Problem is, this treatment has exacerbated my rosacea to the point that I can`t even exercise without burning and flushing. I would like to finish out the melasma treatment as the results have been great. Is there something I can add to the mix to address the rosacea (while continuing the tret/HQ)? Would switching to Triluma be better on the Rosacea (as compared to just tretinoin and HQ)? Also, I understand that IPL can exacerbate melasma. Is this true if the IPL machine is set at settings for rosacea? Perhaps I should proceed with IPL while I`ve got the HQ on board? I guess this is what I get for wanting it all :-) Thanks for your help!
Answer: Avoid the creams on the Rosacea and try getting some IPL or Fotofacials on the Rosacea to control it. I do not think Triluma or Retin-A will help Rosacea and will likely aggravate it. IPL can short term aggravate the melasma, but will help long term. The settings for red and brown on the IPL need to be different.JG 2.20.12
- Thank you dr Garcia. To clarify the aspirations has had no blood only serous fluid that appears clear with no infection. I am sorry I did not clarify that earlier. I wonder if warm compress would be better than ice? With this additional info should I consider asking about a drain?
Answer: Unusial to be clear fluid with no blood, could be an injury to the saliva gland causing it. I do think more surgery might stlll be needed. I would warm at this point. JG 2.19.12

- Dear dr Garcia. I had a facelift 5 days ago and the result on right side is healing beautifully although more swollen than my ps would like. The left however is very swollen and painful at the jawline. My ps has drained it now three times with 5-7 ccs removed each time. It does not seem to have improved and I am wearing compression garment quite tight. She is very qualified but I am now increasingly worried. Should I seek further advice? I have followed your blog for some time and this was not a complication I had ever heard of for the face-- more for a tummy tuck. Any advice is greatly appreciated.
Answer: Repeated aspirations with that amount of output might force a return to the operating room to identify and stop the bleeding.if it continues, that side will not heal as well or as quickly. You might to ask him at what point he is considering returning to the operating room. All the best. JG 2.19.12

- Had Erbium resurfacing 2 mos ago, raised vertical scars under both my eyes. Doc wants to see me in 6 mos. I don`t think this will heal in 6 mos. I am so scared that my face is ruined. Is this going to heal in 4 more mos? If not is there any treatment for this?
Answer: If the scars are raised and vertical you should have at least so me kenalog or 5FU shots to decrease the scarring . JG 2.19.12
- Hi Dr Garcia, Am I to use hydroquinone up to the day of my PCA peel? How many days after the peel do I resume the hydroquinone? I have the same questions regarding the tretenoin.
Answer: We use both until the day of the peel and start between 5-7 days afyer. JG 2.19.12
- The skin on my hands are so thin and wrinkly - what fillers-lasers would help????
Answer: I personally think lasers on the hands are risky. As to fillers, the ones like Juvederm will last a very short period of time and I have seen granulomas with Sculptra and Radiesse injections. Sorry. JG 2.17.12

- Hi, I had a revision rhinoplasty approximately 19 months ago. Overall it looks good, but still seems to swell intermittently. This seems to be in conjunction with a stuffy feeling internally. Could inflammation in the lining of the nose from an allergy or cold cause the nose to appear swollen or thicker externally? Thanks.
Answer: yes, internal swelling can cause that, so be sure to take whatever medications or sprays you use for that issue in order to control it. JG 2.16.12
- Dear Dr garcia, I hope all is well with you, thank you so much for taking the time to answer my question. I am thinking of excising my malar mounds. But before i do this i would like to equip myself with as much knowledge as possible. You are the only Dr who is honest and has a conscience as far as i am concerned so i know for certain that you will be honest with your advise. My questions are : How long will the healing take? Will the stitches have to removed manually or will they be the dissolvable type? How long will it take for the scars to fade away ? is the procedure done under local anaesthetic or general and how long does it take? Should i see a plastic surgeon or a dermatologist? Although i am in the UK and i know the charges will somewhat be different, if you were performing this procedure how much would you charge for the excision under both eyes? Last of all---Is there any other way to get rid of the malar mounds other than excision? perhaps some new procedure? I hope to hear from you. Answer: the healing takes time but in a couple of weeks, things look pretty good. Any sutures placed on the outside are usually manually taken out in 5-7 days. There will be absorbable sutures below the skin that will dissolve on their own. The complete fading of the scars is one year, but by 6 weeks the look pretty good. The procedure can be done under local anesthesia, yes. The procedure can take 30-45 minutes or so. I think a plastic surgeon would be better for this particular issue.There are really no other way to remove them. I would estimate the fees to be between $1500 and $2000. - Hello, I currently use retin a at night and a sulfur cream in the morning. I have been using 20% vitamin c serum under my eye area only in the mornings but would like to begin using it over my entire face. Is it ok to layer this under the sulfer cream? Thank you.

- Regarding treatment after chemical peel, what are the non-petroleum based creams you use and for how long? Where do I get squaline oil? Does anything take the red away faster and do you recommend stopping vitamins, fish oil, etc. before? Thanks so much.
Answer: we like NeoCutis High Protein, the squalene can be purchased at any large health food store. The redness is a matter of time, for sure do not use any steroid creams.I do not have the patients stop their supplements prior to a chemical peel.All the best. JG 2.16.12
- Will a jowl lift get messed up if one has surgery to shave down chin a few months later? I do not know if the jowl lift will be lost when moving shaving down chin/jaw area? Also, can a jowl lift and shaving down chin be done on same day. If the surgeries on done on different dates, is it better to have the jowl lift or chin shaving surgery first? I know the jowl lift can be done in office with numbing shots, but I do not know if chin shaving can be done in office, too?
Answer: surgery on the chin might cause some swelling in the surrounding areas but should not affect the outcome of the previous procedure. I think a surgical lifting of the jowl and the chin surgery could be done at the same setting, yes. If done separately, it really will not matter much which one goes first. It would be hard to shave the chin bone down under local anesthesia. JG 2.16.12
- Dear Dr. Garcia, you`ve always been my go-to guy (and wonderful with your time and information). This is LONG, so please forgive me but I need to give you the entire backstory .... I`m 46. Back in October, I had my upper and lower eyelids done. I had them done in the past with good results. This time, not really. About 6 years ago, I (stupidly) went to a different PS (because my normal PS was on vacation for 3 weeks). The untested PS put Radiesse above my cheekbones but below my eyes. Disaster! For more than THREE years I had little lumps under my eyes - which I believe GREATLY attributed to them getting baggy (more so than my age). My normal PS wouldn`t even consider doing my eye surgery for almost 4 years because of it (he didn`t want someone else`s mistake becoming his). Finally, last year, (to my dismay and joy - dismay because he said I needed the eyelift, joy because he would finally do it) I got the procedure. Within a month I felt there was very little improvement (the undereye bags seemed to return AND a droop/bag - over my RIGHT eye specifically - came back as well). I brought it up to him shortly thereafter. He told me to give it another 2 months. So I did. No improvement. A week ago, he agreed to do a little more work (in his office, ugh!) But he does not want to do the incision under my eye but rather out to the side. He said he didn`t want to put more tension under my eyelid (though I think he could but I`m not the doctor and therefore look to people such as yourself and him to determine these things). He wants to put the incision out by the corner of my eyes - thereby making it noticeable. I honestly need your help in making this decision. I spent a lot of money on this procedure and want the best result but a scar off to the side? Please tell me your thoughts. I don`t think he will do the incision under my eye. Frankly I don`t believe he`ll do anything unless it is out to the side. Thank you!!!
Answer: approaching the fat pockets can be done on the corner, but accessing any fat pockets close to the nose is difficult and done blindly.Although going through the same incision can, in certain circumstances, create problems, those problems are due to scar tissue formation or overzealous skin removal.If you are meticulous with bleeding control and excise no skin, the risks are very small. There is also an operation called a transconjunctival approach where the fat pockets are removed with an incision inside of the eyelid, works well for fat pocket removal and leaves no external scar and no skin can be removed. Usually the transconjunctival eyelid surgery is done with some sedation with local anesthesia and the full lower lid incision re-opening would be more comfortable with some sedation along with the local anesthesia too. - What do you recommend doing before and after a phenol chemical peel to get the shortest recovery....what to use after.?
Answer: Retin-A and hydroquinone for 2-4 weeks before, after use the ointment the doctor gave you until the surface is healed, we use non-petroleum based creams, and can switch to squalene oil once skin is intact, as a moisturizer, also sunblock after skin is healed. JG 2.16.12
- Good evening, Dr. Garcia. I am undertaking a series of Skin Medica peels. For optimal results, the aesthetician in my plastic surgeon`s office is recommending doing them every 2 weeks. Does that sound like the right amount of spacing to you? Thank you for helping to keep us all safe.

Answer: I think any laser that causes skin sloughing cam lead to problems on the hands. JG 2.14.12
- Hello Dr, i read you use sculptra in the neck area,How would that work and where would it be injected at?
Answer: It is injected diffusly into the skin to create new collagen. JG 2.14.12
- Do you recommend prevelle for fine lines under the eyes and in the festoon bad area? If not what do you recommend? Thanks so much?

- Hello Dr Garcia I am a 44 years old and I am trying to keep from going under the knife, I am beginning to see some little signs of turkey type loose skin in my neck and I was wondering what you thought of excillis for tightening and fat removal. I have read that if given enough treatments that it might help with this problem and if not what you recommend, cause I am willing to do maintenance type procedures. Should I stick with the palomar fraxel try excillis, I dont want to waiste money. Thanks so much in advance.
Answer: I am unimpressed by the results I have seen from that technology. I think the Palomar Fraxel will also yield little once the swelling is down. I personally do not feel there is much non-invasive on the neck that yields enough of a change to warrant the expense. JG 2.14.12
- Hi Dr. G. How long does Artefill last? I was told Radiesse would last 12-18 months, and Restylane 6 months. Turns out Radiesse only lasts about 3 months for me. Thank you.
Answer: Artefill is intended to last for years but it depends on what you are using it for. The filler might happen to last for years, but as it is not stopping the aging process, there are areas, such as a nasolabial fold, which will continue to age regardless of the filler employed and more filler will be needed in the future and eventually surgery. The length of time a filler lasts does not mean that the correction for which it was used will last the same amount of time. JG 2.13.12
- Hi Dr. How long does Radiesse last? Do you use Sculptra to fill in the undereye area? My doctor says he uses a method that does not leave bump. Thank you.
Answer: in my experience it lasts about as long as Juvederm or Restylane, so it depends on where you inject it, but on average 2-4 months or so. I do not use Sculptra for the tear trough as the skin there is thin and you can be left with lumps. I am sure your doctor is trying his best, so ask him how will he fix the problem with the lump if it occurs and just as importantly, what will he do it the Sculptra does not adequately fill in that area. JG 2.13.12
- Hello Dr., This is a follow up to the Qs on Artefill. How long does Aretefill last? Your answer that that the skin loosening that is causing the folds to be visible will progress and the folds will return although the filler is still there. So what does that mean? Will I just need more filler when this happens?
Answer: correct, and at some point, no matter how much filler you use, it will not correct the fold and surgery will be required. JG 2.13.12
- I am trying to do my homework after being burned by treatments that don`t work... so, I decided once again to pick your brain. In Pittsburgh, there is a limited variety of laser treatments available. Which one do you believe will improve wrinkles and skin texture, Fraxel II, Fractional 1540, Refirme, Titan?? Thanks so much.
Answer: on that list, I would say Fraxel II, but have mild expectations, all the best. JG 2.13.12

Answer: No, they do not add up, they dilute one another. JG 2.12.12

- Yes, but the physician who has to perform the surgery for me needs to know where to get the silicone temporal implants, he can perform the surgery but its been a while since he`s done those, so he needs to familiarize himself with those companies thanks.

- Good Morning Good Doctor! Well, after all the worrying and freakin` out about doing the 25 %TCA on my face, so far, kinda dissapointed. I did it last Thursday and only have a little flaking. The TCA was at least 4 years old, does it lose its strenght? I still would like to improve the wrinkles and remove the 2 `OPS`s (olp people spots ha, humor). What is effective and safe when it comes to peels? My husband is worried that I will end up with thinner and thinner skin. When can I give it another try? Thanks so mucha as always and stay warm! Answer: typically chemicals are good for 1-2 years, less time if they are made by a compounding pharmacy.I think seeing a physician for a deeper TCA peel or a laser would be better. It is not uncommon to require a series of sessions though. JG 2.12.12
